W-9 form (.gov) - minnesota.gov

The Energy Assistance Program is a federal program offered to low-income individuals and households to fund energy purchases. A person's household income is the amount of money they earn by working full-time.  The Federal government provides 100% of the cost of heating fuel as a part of the Low Income Home Energy Assistance Program (LIH EAP). Individuals should contact their local Public Service Agency to apply for assistance.  To receive assistance, an applicant must meet four simple requirements: 1) They must live in the household. 2) The total household energy usage must be less than the household budget (energy bill). 3) The energy used must be the result of the household's own activities; i.e. the furnace, the washing machine, etc. 4) The household must meet their actual housing budget. An applicant may qualify to receive financial assistance if they meet all three of these requirements.  If all four of these are met, an applicant may be eligible.
